Job Summary
Our client is seeking a Clinical Nurse Specialist/Registered Nurse with a focus on transitioning patients from inpatient to home care. The primary responsibilities include promoting home infusion services, educating patients and staff, coordinating care transitions, and building relationships with referral sources to ensure seamless discharge planning.
Key Responsibilities
- Transition patients from inpatient to home care while remaining stationed at the facility.
- Promote services to discharging patients as a sales professional.
- Educate patients, families, and staff on home infusion services.
- Assess patient eligibility for alternative site care.
- Coordinate care transitions and discharge planning.
- Maintain relationships with referral sources like case managers and providers.
- Collaborate with Account Managers to increase referral volume and meet sales goals.
- Review medical records and gather necessary pre-admission and insurance information.
- Facilitate communication across care teams for effective treatment plan execution.
- Support patient education and foster therapy independence.
- Coordinate logistics for supply orders and care setup.
Qualifications
- Bachelor's of Nursing.
- Licensed RN or Pharmacist in the state of practice.
- Minimum 2+ years of nursing experience.
- Experience in building relationships.
- Knowledge of healthcare standards such as ACHC, URAC, etc.
- Strong customer service skills.
- Proficiency in Microsoft Office (Word, Excel, PowerPoint).
- Ability to identify and improve operational processes.
- Reliable transportation with valid driver’s license and insurance.
Preferred Skills
- Experience in home infusion or alternative site care.
- Experience in discharge planning.
- Experience in growing provider partnerships.
Schedule/Shift
Monday to Friday, 8:00 am to 5:00 pm. No weekends.
