Job Summary
We are seeking a detail-oriented and organized Medical Records Clerk to join our Health Information Management team. This role is the backbone of our digital infrastructure, responsible for the conversion of physical patient documentation into our Electronic Health Record (EHR) system. You will ensure that every piece of medical data is accurately prepped, scanned, indexed, and filed, maintaining the highest standards of patient confidentiality and data integrity.
Key Responsibilities
- Document Imaging & Scanning: Operate high-volume scanners to digitize paper records, ensuring clear image quality and correct orientation.
- Quality Assurance: Review scanned documents for legibility and completeness. Re-scan documents as necessary to maintain 100% accuracy.
- Indexing: Accurately assign scanned documents to the correct patient encounter, document type, and folder within the EHR (e.g., Epic, Cerner, or Meditech).
- Record Preparation: Remove staples, tape, and paper clips; repair torn pages; and organize documents in chronological order prior to scanning.
- Data Entry & Clerical Support: Update patient demographics, process Release of Information (ROI) requests according to hospital policy, and answer departmental phone calls.
- Compliance & Privacy: Strictly adhere to HIPAA regulations and hospital policies regarding the privacy and security of Protected Health Information (PHI).
- Physical Filing: Manage the short-term storage and eventual secure destruction of physical records following digital verification.
Qualifications
- High School Diploma or equivalent required.
- 2+ years in a clerical/administrative/front desk role (open on industry).
