Job Summary
Our client is seeking a Non-Clinical Case Manager to provide a wide range of medical and dental administrative support and case management services. The primary focus is on managing the medical readiness of Service Members through care coordination, tracking medical documentation, and educating soldiers on available resources to ensure compliance with health standards.
Key Responsibilities
- Manage Service Member care from the initial identification of a condition until they are medically ready or administratively separated.
- Coordinate treatment plans with medical facilities and request specific exams and evaluations.
- Ensure consistent follow-ups by the case management team for Service Members referred by providers.
- Maintain automated referrals and submit medical documentation to military or contracted providers.
- Build initial shells for temporary profiles in the E-PROFILE database within five business days of receiving documentation.
- Support Periodic Health Assessments, Soldier Readiness Processing, and Certification events.
- Attend PHA events to support medical readiness and track the accuracy of health assessments.
- Educate Service Members on resources like TRICARE benefits and Veterans Affairs eligibility.
- Maintain medical documentation and charting in the eCase system and update documents in the MEDCHART HRR module.
- Add medical documentation to the Soldier Treatment Record as per AR 40-66 standards.
- Compose Narrative Summaries in memo format within 10 days of profile determination.
- Generate monthly reports for the Contracting Officer Representative on case statuses.
Qualifications
- Must be a Registered Nurse
- Strong technical skills with proficiency in systems like MEDCHART, eCase, E-PROFILE, and AHLTA (training provided)
- Adhere to HIPAA and PII protection standards
