Job Summary
The Clinical Documentation Improvement (CDI) Specialist is responsible for reviewing medical records to ensure accurate, complete, and compliant clinical documentation. This role works collaboratively with physicians, nurses, and other healthcare professionals to improve documentation quality, support appropriate coding and reimbursement, and reflect the true severity of illness and risk of mortality of patients.
Key Responsibilities
- Review inpatient and/or outpatient medical records to assess accuracy, completeness, and specificity of clinical documentation
- Identify documentation gaps and issue compliant queries to providers for clarification
- Ensure documentation supports appropriate coding, case mix index (CMI), and reimbursement
- Collaborate with coding, quality, and revenue cycle teams to align documentation practices
- Educate physicians and clinical staff on documentation best practices and regulatory requirements
- Monitor key performance indicators (KPIs) such as query response rates, agreement rates, and documentation quality metrics
- Maintain compliance with federal, state, and organizational guidelines (e.g., CMS, HIPAA)
- Participate in audits and support external review processes
- Stay current on coding updates (ICD-10-CM/PCS), payer regulations, and industry standards
Qualifications
- CCS, CCDS or RHIA is required
Experience
- Around 5 years of clinical nursing experience
- 2 plus years of CDI experience
