Job Summary
Our client is seeking a dedicated Patient Care Coordinator to serve as the essential link between patients and the healthcare system. The primary goal is to ensure that patients receive the care they require and thoroughly understand their medical conditions, medications, and any instructions provided.
Responsibilities / Job Duties
- Plan, coordinate, and carry out patient care operations.
- Address patient requests and needs in a professional manner.
- Respond to patient concerns timely and ensure patient satisfaction.
- Schedule patient appointments and make reminder calls.
- Greet patients and direct them to examination rooms.
- Maintain patient medical records up-to-date.
- Obtain insurance information from patients, process patient billings, and collect payments.
- Collect and maintain patient health, demographic, insurance, and financial information.
- Develop a patient care plan based on Physician’s assessment information.
- Suggest process improvements to enhance patient services.
- Maintain patient records confidential and secure.
- Arrange for insurance pre-certifications and patient registration prior to patient services.
- Maintain patient records in an organized manner according to accreditation regulations and departmental protocols.
- Participate in and contribute to departmental performance improvement activities.
- Maintain good working relationship with medical directors, physicians, office personnel, and other hospital department employees.
- Ensure all communications reflect the standards and mission of the organization.
- Maintain patient confidentiality and follow HIPAA guidelines.
- All other duties as assigned.
Experience
- Minimum 1 year of experience in a clinical position with demonstrated competency in performing required skills.
- Pulmonary experience preferred.
Skills
- Strong interpersonal and communication skills with patients, families, physicians, and healthcare team members.
- Ability to foster optimal quality of outpatient care.
- Excellent written and verbal communication skills.
- Proven ability to develop rapport and maintain positive, professional relationships with diverse patients, staff, and physicians.
