Job Summary
Our client is seeking an Outreach Coordinator to collaborate with the Stars and Risk team, focusing on the collection and review of medical records. The coordinator will perform HIPAA-compliant outreach to patients to address care gaps, support HEDIS and CAHPS initiatives, and work with cross-functional teams to improve health outcomes.
Responsibilities / Job Duties
- Collect and review medical records in partnership with the Stars and Risk team; conduct provider outreach and ensure accurate data entry.
- Perform HIPAA-compliant outreach to patients to address care gaps, including medication adherence and preventive services.
- Support HEDIS, CAHPS, and other quality initiatives through targeted member engagement and documentation.
- Identify barriers to care and collaborate with cross-functional teams to improve health outcomes.
- Coordinate with providers, vendors, and care team members, including facilitating warm transfers as needed.
- Develop training materials and maintain documentation to support quality improvement efforts.
Minimum Education and Experience Qualification Requirements
Education
- High School Diploma
Qualifications
- Minimum of two (2) years of experience in a health plan, managed care, or similar environment.
- Experience with electronic medical records (EMR) systems preferred.
- Knowledge of CMS Star Programs and HEDIS strongly preferred.
- Willingness to travel to provider offices as needed.
- Valid driver's license, reliable transportation, and current auto insurance with a clean driving record required.
Skills
- Experience coordinating projects and managing timelines.
- Strong organizational skills with high attention to detail.
- Excellent written and verbal communication skills, including proofreading and grammar.
- Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint).
- Familiarity with HEDIS technical requirements preferred.
Additional Requirements
Must be fluent in either Korean, or Vietnamese.
Schedule / Shift
Standard Business Hours, Monday-Friday 8am - 5pm.
