Job Summary
Our client is seeking a compassionate and skilled Primary Care Nurse Practitioner to join a dedicated healthcare team. The role involves delivering high-quality, patient-centered care to a geriatric population across multiple care levels, including primary care, subacute, long-term care, and assisted living. Responsibilities include conducting assessments, managing health conditions, and collaborating with an interdisciplinary team.
Key Responsibilities
- Conduct thorough assessments and physical examinations of senior residents.
- Diagnose and manage acute and chronic health conditions.
- Develop, implement, and monitor individualized care plans.
- Prescribe medications and treatments as appropriate.
- Provide health education and counseling to residents and their families.
- Collaborate with physicians, nurses, and interdisciplinary care team members.
- Maintain accurate and up-to-date medical records.
- Participate in care team meetings and quality improvement initiatives.
- Ensure compliance with all regulatory and organizational policies.
- Perform IV insertion if needed (infrequently).
- Support efforts to prevent unnecessary hospitalizations and readmissions.
Qualifications
- Master’s degree in Nursing from an accredited program.
- Current Nurse Practitioner license in the state of New Jersey.
- About 5 years of experience in a primary care or geriatrics setting.
- Recent subacute or acute care experience required.
- Experience in long-term care and medical group settings preferred.

Schedule/Shift
Monday, Wednesday, Friday | 8:30 AM – 5:00 PM. Could also add a Tuesday or Thursday at a different location.
