Job Summary
We are seeking a dedicated Licensed Practical Nurse (LPN) for our client to provide direct and indirect patient care, coordinate care transitions, engage in patient communication, and support clinic operations. The successful candidate will work in a mission-driven environment focusing on care coordination and transitional care, involvement in complex and high-risk patient cases, and collaboration with care teams.
Key Responsibilities
- Deliver direct and indirect patient care in clinic settings.
- Administer medications, injections, and other treatments.
- Conduct vital signs, triage, and patient assessments.
- Assist healthcare providers during procedures.
- Manage and coordinate post-discharge follow-ups and transitions.
- Identify and escalate high-risk patient cases appropriately.
- Conduct patient calls, inquiries, and telephone follow-ups.
- Educate patients and families on care plans, medications, and treatments.
- Monitor clinic flow and ensure efficient operations.
- Order laboratory tests, process specimens, and maintain clinic supplies.
Qualifications
- Active Washington State LPN license.
- Completion of an LPN program.
- Minimum of 1 year of LPN experience.
- Proficiency with electronic health records (EHR).
- Strong knowledge of nursing practices, clinical procedures, and medical terminology.

- Technical proficiency in EHR systems and medical documentation.
- Effective communication and strong teamwork skills.
- Compassionate patient care with a focus on patient education.
- Ability to handle lab work and maintain infection control protocols.
