Job Summary
Our client is seeking a dedicated Nurse Practitioner to join their team, focusing on a Value-Based Care Model to serve the senior population. The primary goal is to foster relationships with patients and work collaboratively alongside a doctor.
Key Responsibilities
- Prescribe medication and address non-complex cases.
- Engage in value-based care, aligned with Medicare Advantage.
- Participate in an 8am team huddle and meet with the first patient by 8:30am.
- Manage longer appointment times with fewer patients compared to most groups.
- Utilize EMR: eClinical Works for patient records.
Qualifications
- Possession of BLS or ACLS Certification.
- Family Nurse Practitioner License or in the process of obtaining one.
- Several years of primary care experience, including geriatrics or VBC experience.
Experience
- Experience working with a geriatric population.
- NP experience within Integrative Medicine, Family Practice, Geriatrics, or Pain Management.
- Value-Based Care experience preferred.
Skills
- Technical proficiency with EMR systems, specifically eClinical Works.
- Strong communication and relationship-building skills.
- Ability to work autonomously and collaboratively in applicable states.
Additional Requirements
- Full-time position with a schedule from Monday to Friday, 8am-5pm.
