Job Summary

Our client is seeking a Nurse Practitioner to join their team, focusing on serving the senior population within a Value Based Care Model. The primary goal is to build strong relationships with patients and work alongside doctors to ensure high-quality care.

Key Responsibilities

• Prescribe medicine and handle non-complex cases.
• Maintain a full daily schedule which includes team huddles, patient consultations, and administrative duties.
• Provide care aligned with Medicare Advantage and Value Based Care principles.
• Ensure longer appointment times to see fewer patients than typical groups.

Qualifications

• BLS or ACLS Certification required.
• Must possess or be in the process of obtaining a Family Nurse Practitioner License.
• Several years of Primary Care experience (including Geriatrics) or Value Based Care experience.
• Experience working with a geriatric population, integrative medicine, family practice, geriatrics, or pain management is preferred.
