Job Summary
Our client is seeking a dedicated Hospice Social Worker responsible for conducting comprehensive psychosocial assessments, developing individualized treatment plans, and coordinating care across providers. The role involves delivering both individual and group counseling services, education for patients and families, case management, and discharge planning while maintaining accurate and timely documentation.
Key Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized treatment plans.
- Provide counseling, therapy (individual/group), and patient/family education.
- Reassess and update treatment plans based on patient progress.
- Deliver case management services and coordinate care across providers.
- Assess Social Determinants of Health and connect patients to resources.
- Collaborate with patients, families, and care teams on discharge planning.
- Maintain accurate and timely documentation in compliance with policies and regulations.
- Respond to behavioral health and medical emergencies as part of the care team.
- Ensure patient confidentiality in accordance with legal and regulatory standards.
- Refer patients to appropriate community agencies and support services.
- Tailor care based on patient population needs (age, culture, language, etc.).
Qualifications
- Master’s Degree in Social Work (MSW)
- Active New Jersey Licensed Social Worker (LSW)
- At least 1 year of social work experience
- Strong ability to conduct psychosocial assessments and develop treatment plans
- Knowledge of crisis intervention theory and application
- Understanding of disease processes, behavioral health, and treatment techniques
Skills
- Proficiency in Microsoft Office and/or Google Suite
- Strong written and verbal communication skills
- Ability to collaborate effectively within a multidisciplinary team
- Experience with case management and discharge planning
- Ability to assess and address Social Determinants of Health (SDOH)
