Job Summary
The ICU Registered Nurse is a critical clinical leader within our primary care team. Serving as the primary liaison between patients and providers, the RN integrates advanced clinical judgment with efficient care coordination. This role is designed for a Registered Nurse who excels at high-level triage, complex patient education, and overseeing the clinical competency of the medical support staff to ensure seamless, high-quality care delivery.
Key Responsibilities
- Utilize the Nursing Process to assess and triage patients for the appropriate level of care as the clinical "air traffic controller" for both scheduled visits and emergent walk-ins.
- Identify and support "rising risk" patients, developing care plans that address physical, psychosocial, and social determinants of health.
- Support the professional development of the clinical team by assessing and validating the skills and competencies of Medical Assistants (MAs).
- Work in tandem with a dedicated team of 3 APPs and 1 Physician, handling complex paperwork, specialty referrals, and standing order protocols to optimize provider efficiency.
- Provide expert education on complex disease processes (e.g., diabetes, hypertension) and medication regimens, fostering patient engagement and self-management.
Qualifications
- Current Colorado RN License.
- Current CPR (BCLS) certification.
- Graduate of an accredited Nursing Program required; BSN preferred.
- Two years' experience in case management and clinical nursing preferred.
Experience
- Two years' experience in case management and clinical nursing preferred.
- Basic computer skills are necessary, including the ability to access the intra/internet, manage timecards, review policies and procedures, and communicate through email. Proficiency in basic data entry, PowerPoint presentation creation, and document formatting is required.
Skills
- Proficiency in using Epic software is preferred.
- Spanish bilingual skills are desirable.
- Experience with Medicare/Medicaid populations and paperwork is beneficial.
Schedule/Shift
- M-F, no weekends.
- 4 10-hour shifts (7:30am-6pm) or 5 8-hour shifts (8am-5pm).
- Day off may vary depending on clinic demand.
