Job Summary
Our client is seeking a skilled Residential Nurse Case Manager to support and enhance the overall health and well-being of individuals in their care. The primary responsibility of this role includes the coordination of services, advocacy for individual needs, and education to ensure high-quality healthcare outcomes. The Nurse Case Manager will work closely with individuals with developmental disabilities, brain injuries, and neurological conditions to contribute to consistent and effective care within the program.
Key Responsibilities
- Act as a central point of contact, advocate, and informational resource for both routine and specialized healthcare needs.
- Promote a collaborative, team-based approach to delivering high-quality care.
- Evaluate and monitor ongoing, emerging, and complex healthcare needs of individuals served.
- Partner with interdisciplinary teams to design and implement comprehensive care plans and service systems.
- Ensure coordination across providers and promote the integration of healthcare services.
Qualifications
- Must be a Registered Nurse (RN) with an active and valid New Jersey license.
- Associate’s or Bachelor’s degree in Nursing required.
- Certification in Case Management, Developmental Disabilities, Brain Injury, or as a Certified Rehabilitation Registered Nurse (CRRN) is a plus.
Experience
- At least two years of general nursing experience is required; candidates with three or more years of relevant experience are preferred.
Skills
- Strong communication and coordination skills.
- Ability to work effectively with interdisciplinary teams.
Additional Requirements
- Monday to Friday, 8am-4pm.
