Field Nurse Practitioner (NP) – In-Home & Community Care
Full-Time | Flexible Schedule Options | Autonomy + Travel-Based Role

We are seeking a Field Nurse Practitioner (NP) to deliver high-quality, preventive and primary care services directly to patients in their homes and community settings. This is a fully mobile, patient-centered role ideal for providers who value autonomy, flexibility, and making a direct impact in underserved populations.
This position focuses on comprehensive health assessments and preventative care, allowing you to spend meaningful time with patients in the environments where they are most comfortable.
Job Responsibilities
- Conduct in-home and community-based visits, including private residences, skilled nursing facilities, and pop-up clinic settings
- Perform comprehensive assessments (history & physicals) to identify care gaps and preventative needs
- Address acute and chronic conditions while determining appropriate level of care
- Develop and document diagnoses and care plans
- Order and interpret diagnostic tests (labs, imaging) as appropriate
- Coordinate care, including referrals to specialists when needed
- Provide post-discharge support to reduce hospital readmissions
- Prescribe medications and treatments within scope of practice
- Document all patient interactions in EMR (completed in the field)
- Participate in community outreach through mobile or pop-up clinic initiatives
- Collaborate with other providers and participate in team meetings
Role Structure & Expectations
- ~45-minute patient visits focused on initial comprehensive assessments
- 6–9 visits per day depending on schedule
- Travel within a ~60-mile territory (up to ~120 miles/day)
- No ongoing patient panel or follow-up care required
- Field-based documentation using EMR
Schedule Options
Option 1: 4x10 Schedule
- Monday–Friday (4 days/week) | 8:00 AM – 7:00 PM
- Includes 1 Saturday per month
- 8–9 visits per day
Option 2: 5x8 Schedule
- Monday–Saturday (5 days/week)
- 3 days: 8:00 AM – 5:00 PM
- 2 days: 10:00 AM – 7:00 PM
- 6–7 visits per day
- Consistent weekly schedule
Qualifications
Required:
- Active Nurse Practitioner (NP) license (Family or Adult)
- Comfortable working in a 100% field-based role
- Ability to travel daily within assigned territory
- Must reside within or near assigned region
- Ability to complete 5-week onboarding/training program
Preferred:
- Experience in home health, hospice, or community-based care
- Background working with underserved or high-risk populations

Compensation
- Salary range: $110,000 – $160,000 annually
