Job Summary
Our client is seeking a dedicated Hospice Social Worker responsible for conducting psychosocial assessments, providing end-of-life counseling and emotional support, coordinating community resources, and assisting with placement coordination. The position primarily involves fieldwork with some remote documentation.
Key Responsibilities
- Conduct psychosocial assessments for hospice patients and families
- Provide end-of-life counseling and emotional support
- Coordinate community resources such as housing, Medicaid, and caregiver support
- Assist with placement coordination to assisted living or skilled nursing facilities
- Participate in interdisciplinary care team meetings
- Complete documentation in Homecare Homebase EMR
- Travel to patient homes and facilities across the service territory
Qualifications
- LSW, LMSW, LCSW, LPC, or LCPC license required
- Active Illinois social work/counseling license
- Hospice, palliative care, or medical social work experience
- Ability to conduct psychosocial assessments
- Reliable vehicle and valid driver's license
- Comfortable with EMR documentation
Experience
- Experienced with Homecare Homebase (EHR)
- Experience working with Medicaid/low-income populations
- Skilled in nursing/assisted living placement experience
Skills
- Able to work independently with minimal supervision
- Strong communication and collaboration skills
- Experience in interdisciplinary team settings
Additional Requirements
- Bi-weekly interdisciplinary meetings (4 meetings every two weeks)
- Field visits during daytime with remote documentation when not visiting patients
