Job Summary
Our client is seeking a Primary Care Physician to provide comprehensive coordinated medical care within the PACE benefit framework. The role involves conducting assessments, collaborating with an interdisciplinary team, updating care plans, and providing primary care treatment to program participants.
Responsibilities / Job Duties
- Provides comprehensive coordinated medical care as guided by the PACE benefit.
- Conducts comprehensive initial, 6 month, annual, and periodic assessments of program participants.
- Participates with interdisciplinary team in development and implementation of participants’ care plans.
- Reviews and signs care plans every 6 months.
- Collaborates with and supervises nurse practitioners and physician assistants as appropriate.
- Updates care plans per policies and procedures as new problems arise.
- Participates in IDT discussions, care conferences, and family meetings as needed.
- Provides primary care assessment and treatment to program participants.
- Oversees and orders most diagnostic tests and medications.
- Coordinates care of hospitalized participants with hospitalist physicians.
- Guides appropriate use of hospital and emergency department services and coordinates discharge.
- Assists with clinic and administrative staff to keep clinic function smooth and efficient.
- Maintains safe working environment and confidentiality of procedures.
Minimum Education and Experience Qualification Requirements
Education
- Physician holding an M.D. degree and a current, unrestricted license to practice medicine in California.
- Formal training in Internal Medicine or Family Practice to meet requirements for board certification.
- Current certification in Basic Life Support.
Qualifications
- 1+ years experience working with geriatrics.
- Interest in working with the frail/elderly.
- Drug Enforcement Agency Certificate or ability to obtain upon job offer.
- PACE experience (Preferred).
Schedule / Shift
M-F 8am-5:30pm
