Position Overview
We are seeking Nurse Practitioners and Physician Assistants to conduct Initial Health Assessments (IHAs) for Medicare and Medicaid members in Syracuse, NY. This role is primarily field-based, completing in-home patient visits with the flexibility to create your own schedule.
This opportunity is ideal for providers looking for supplemental income, flexible hours, or part-time clinical work. Assessments typically take 15–60 minutes and focus on preventive care, health history, and overall wellness evaluations.
Key Responsibilities

Conduct comprehensive health histories and physical assessments in patients’ homes

Review medications and evaluate overall health status

Identify social determinants of health and provide patient education

Document assessments accurately in the EMR within 24 hours

Travel to patient homes within a 50-minute radius
Compensation
-
$550 for training
-
$100 per in-home assessment
-
$70 per telehealth assessment

$50 compensation for no-shows
Schedule

Fully flexible schedule – choose your own availability

Minimum expectation of 30 assessments within the first month

Qualifications

Active Nurse Practitioner (NP/APRN) or Physician Assistant (PA) license

At least 1 year of clinical experience
