Job Summary

Our client is seeking a dedicated Registered Nurse to join their team. The Quality Implementation Nurse will facilitate quality patient care and coordinate supportive services within the health center. The position involves providing direct clinical support, serving as an EMR champion, and implementing a patient-centered care approach.

Key Responsibilities

• Provide and facilitate competent healthcare for patients and families.
• Competently conduct telephone triage for ages 0-150, including remote assessment and medication management.
• Initiate care coordination within the scope of Pennsylvania nursing licensure when necessary.
• Use computers for accessing electronic health records for data retrieval, entry, and charting updates.
• Provide health and wellness education to individuals and groups.
• Support initiatives to improve quality of care or clinical outcomes.
• Assist providers with direct patient care and procedures.
• Administer immunizations and injectable medications to patients age 0-150.
• Ensure all patient documentation is current and complete.
• Coordinate care for patients with chronic diseases.
• Assist in the education and training of staff.

Qualifications

• GED or high school diploma required.
• Active PA RN License.

Experience

• Prior Case Management experience.
• Prior phone triage experience.
• Patient education experience.

Additional Requirements

• Consistent Monday to Friday schedule from 8am to 4:30pm.
