Job Summary
Our client is seeking a Care Coordinator to coordinate health and human services related to patient care, serving as the main point of contact for assigned care teams and their patients. The position involves professional communication with providers, monitoring voicemails, processing referrals, and assisting with scheduling and clinical documentation.
Key Responsibilities
- Coordinate all health and human services related to patient care.
- Serve as the main point of contact for assigned care teams and their patients.
- Communicate professionally with providers.
- Monitor and follow up on voicemails; perform phone triage.
- Process referrals and pre-authorizations & track and follow up on referral statuses.
- Request records from referring providers and outside institutions.
- Provide support with clinical documentation.
- Assist with scheduling follow-up visits and other appointments.
- Help patients navigate the healthcare system and assist in identifying and flagging high-risk patients in the EHR.
- Handle an estimated 30 calls per day.
Qualifications
- 1+ years of Clinical administrative experience
- EHR Experience (Epic Preferred)
- Must be comfortable working with diverse backgrounds as this is a community health center
- Strong communication skills
- Behavioral health experience is preferred
