Job Summary
Our client is seeking a dedicated and skilled Care Coordinator to drive patient engagement, manage complex workflows, and operate across various technology stacks. The role involves outreach calls, managing clinical tasks, and maintaining data integrity while meeting quality and productivity standards.
Key Responsibilities
- Drive patient engagement by conducting outreach calls for care gap closure, appointment scheduling, and chronic condition follow-ups.
- Execute complex workflows including submitting, tracking, and following up on referrals and prior authorizations.
- Pilot new workflow types, document processes, identify edge cases, and provide feedback to teams.
- Operate fluently across platforms, client EHR systems, payer portals, and other systems.
- Meet quality and productivity standards, review AI outputs for quality and accuracy.
Experience
- 3+ years in healthcare operations, such as medical assistant, patient access representative, healthcare call center agent, prior authorization specialist, or care coordinator roles.
Skills
- Strong written and verbal communication skills with the ability to engage patients empathically and professionally.
- Comfortable with technology and quickly learning new platforms and tools.
- Experience working across EHR systems, payer portals, and healthcare SaaS products preferred.
- Ability to thrive in fast-paced and evolving environments.
