Job Title: Nursing Home Surveyor
Locations/Pay (openings for each location):
- Syracuse, Albany: Up to $52/hr
- NYC, Central Islip, New Rochelle: Up to $60/hr
Travel Required by Region:
Syracuse: Broome, Cortland, Lewis, Oswego, Tompkins, Cayuga, Herkimer, Madison, Oneida, Tioga, Chenango, Jefferson, Onondaga & St. Lawrence Counties.
New Rochelle: Sullivan, Orange, Dutchess, Putnam, Rockland, Ulster and Westchester counties
Albany: Albany, Delaware, Fulton, Montgomery, Saratoga, Warren, Clinton, Essex, Greene, Otsego, Schenectady, Washington, Columbia, Franklin, Hamilton, Rensselaer & Schoharie Counties
Central Islip: Nassau and Suffolk counties.
New York City: Bronx, Kings, Richmond, Manhattan, and Queens counties
Overview: These RNs will be doing a mix of standard quality audits, complaint initiated investigations, and more. When they are onsite, the amount of time that they are at the location is dependent on the audit that is required. It is expected that RN Surveyors will be traveling onsite about 85% of the time.
These individuals will be traveling to nursing homes to do state required Recertification (must be done every 15 months) or investigating specific complaints(disease outbreak, falls, etc.).
Travel: 85% of this role is traveling to sites. It is more location based and they will be traveling to the counties (provided separately) that surround their location. If anyone is traveling and not able to return home, they are able to coordinate accommodations through the travel office and miles/ food will be reimbursed at the federal rate. If they are not onsite, they can work from home or in the DOH office.
Job Description: The Nursing Home Surveyor / Complaint Investigator will assist the New York State Department of Health (NYSDOH), Office of Aging and Long-Term Care, with surveillance and investigation activities related to nursing homes (long term care). These activities are mandated to evaluate facility performance and monitor the quality of care and services provided. In this role you will participate as a member of an interdisciplinary survey/investigation team to review allegations of abuse and neglect, perform surveillance activities/tasks at nursing homes (long term care)/Transitional Care Units (TCUs), and Adult Day Health Care Programs (ADHCP) to determine compliance with the requirements of participation; prepare written reports of investigative findings and prepare Statement of Deficiencies (SOD) within required time frame, using Principles of Documentation. participate in emergency surveys at Skilled Nursing Facilities (SNF’s); participate in State monitoring during Immediate Jeopardy situations; as well as other duties.
The position is majority travel and will be onsite at facilities.

- Excellent written and verbal skills with the ability to communicate professionally with co-workers, supervisors, providers, medical and administrative personnel, and residents/patients.
- Ability to translate collected information into a clear, concise, legal, and defensible document.
- Excellent investigative and analytical skills.
- Strong computer skills with working knowledge of Microsoft office products, and the ability to learn new programs.
- Ability to obtain Surveyor Minimum Qualifications Test (SMQT) certification within 12 months of employment, required.
- If approved to work remotely, must have secure internet that meets the speed required to perform all work duties, must have workspace that provides privacy for all work-related calls, and must be able to secure all work-related items when not in use.
- Must have a valid driver’s license & the ability to travel to on-site assignments, required
Education/Experience:
- NYS Licensed and registered, Registered Nurse, required.
- Two (2) years of experience performing utilization review, claims adjudication, medical review, fraud investigation, surveillance or monitoring activities, required; OR Three (3) years of clinical or administrative experience, required; OR A bachelor’s degree in nursing and two (2) years of clinical or administrative experience, required.
- Surveyor Minimum Qualifications Test Certification (SMQT), preferred.
Hours: Monday-Friday 8am-5pm
