Facility Type: PACE (Program of All-Inclusive Care for the Elderly)
Schedule / Shift: Monday – Friday, 8:00 AM – 5:00 PM; No on-call, no weekends, closed on holidays.

As a provider of the Program of All-Inclusive Care for the Elderly (PACE), they focus on a holistic, interdisciplinary approach to senior care.
Job Summary: Our client is seeking a Physical Therapist Assistant to execute physical therapy treatments, monitor non-evaluative data, document records in compliance with regulations, communicate progress, and address participant needs within a rewarding community-based model.
Responsibilities:

Clinical Care: Execute physical therapy treatments as delegated by the Registered Physical Therapist (RPT), including restorative and maintenance program instruction for participants, families, and staff. Lead group exercise programs (8–10 participants) at the Day Center.

Data & Observation: Monitor and record non-evaluative data (vitals, ROM, strength, and mobility progress) to track treatment plan efficacy.

Documentation: Maintain precise, timely records in full compliance with PACE, state, and federal regulations using CareHub and Elation EHR systems.

Communication: Provide regular progress updates to the RPT and Interdisciplinary Team (IDT); participate in care conferences and rehabilitation meetings.

Support & Training: Address psychosocial needs of participants and families; attend required in-services and complete ongoing professional development modules.

Compliance: Adhere to all specific policies and procedures, including conducting home visits if participants cannot attend the center.

Experience in some capacity working with a geriatric patient population.

Must have a valid driver's license, proof of insurance, and reliable means of transportation for potential home visits.

Experience working in a skilled nursing facility (preferred).
Compensation:

Local Rate: $1,600 weekly gross.

Travel Rate: Up to $1,800 weekly gross (includes $1,000 non-taxed stipend for qualifying candidates).

Travel Reimbursement: Up to $350 each way ($700 total) paid on the first and last check.
