Job Summary
Our client is seeking a proactive and empathetic Administrative Assistant, specifically for the role of Case Management Coordinator. You will serve as the "front-line" engagement specialist, working closely with Registered Nurses (RNs) and Clinical Pharmacists. Your mission is to bridge the gap between patients and their care teams, focusing on members managing Diabetes or Renal/Kidney conditions.
As the first point of contact, you will be responsible for identifying patients in need of care, explaining the value of the program, and ensuring they have the resources and appointments necessary to manage their health effectively.
Key Responsibilities
- Member Outreach & Engagement: Conduct 20–25 outbound calls daily to engage members, explain available support services, and overcome barriers to care.
- Clinical Coordination: Review patient charts to identify those with specific A1C levels who are not currently in care. Encourage lab visits and schedule follow-up appointments with Primary Care Physicians (PCPs).
- Data "Hunting & Gathering": Assist RNs and Pharmacists by gathering necessary documents and lab results within the Epic system.
- Resource Navigation: Provide members with essential resources including phone numbers, health education videos, and mailed materials to support their treatment plans.
- Schedule Optimization: Monitor and update clinician schedules by reviewing patient data and removing members whose health metrics have improved and no longer require immediate intervention.
Qualifications
- Proven experience working directly with patients (e.g., Patient Care Coordinator, Patient Navigator, or Financial Counselor).
- High comfort level with telephonic outreach; ability to be persuasive yet empathetic when discussing the importance of clinical follow-ups.
- Strong administrative skills with the ability to learn medical software (Epic) quickly.

- Specific experience in telephonic communication and administrative coordination.

- Technical skills: Proficiency in using software like Epic.

Additional Requirements
- Provides the necessary support for Diabetes or Renal/Kidney care pathways, with an understanding of the dialysis path and the importance of early intervention.
