Nurse Practitioner
Location: Frankfort, KY
Schedule: Monday–Friday | 8:00 AM–5:00 PM EST | 40 hours/week (no on-call or weekends)
Salary Range: $115,000–$130,000 annually

Position Overview
We are seeking a dedicated Nurse Practitioner with wound care experience to provide specialized services to patients residing in a skilled nursing / rehabilitation facility in Frankfort, KY. This role focuses on delivering high-quality wound care, collaborating with interdisciplinary teams, and supporting optimal patient outcomes in a long-term care setting.
The position is ideal for a clinician who is confident in advanced wound care practices, values collaboration, and enjoys working closely with nursing staff and primary care providers.
Responsibilities

Provide comprehensive wound care services to long-term care patients in skilled nursing facilities

Diagnose, treat, and manage complex wounds, including chronic, stalled, infected, diabetic, arterial, venous, mixed etiology wounds, and Kennedy ulcers

Stage pressure injuries in accordance with NPIAP guidelines

Prescribe medications, including controlled substances, in compliance with Pennsylvania law

Order, review, and follow up on treatments, labs, imaging, and diagnostic testing

Collaborate with primary care physicians and interdisciplinary care teams

Communicate regularly with nursing home staff regarding changes in patient condition and care plans

Document care accurately and efficiently using an electronic medical record (EMR)
Required Qualifications

Current, unrestricted Nurse Practitioner license in the state of Kentucky

Board Certification
-
DEA license
-
Clinical background in adult, family, or geriatric care

Strong wound care experience, including advanced and complex wound management

Experience with wound vacs and durable medical equipment (DME), including pressure-relieving devices

Familiarity with advanced wound care products (collagens, alginates, foams, silver dressings, etc.)

Experience working in a long-term care or skilled nursing setting

Ability to work effectively within an interdisciplinary care team

Proficiency with EMR systems
