Job Summary
Our client is seeking a Care Manager who will oversee and ensure the effective development and execution of personalized care plans. This role involves managing interdisciplinary meetings, staffing, and coordination of patient services to improve participant outcomes and satisfaction.
Key Responsibilities
- Review and develop personalized care plans for participants.
- Facilitate and complete Interdisciplinary Team meetings and other care coordination processes.
- Serve as a manager on call during non-operational hours and handle follow-ups for customer issues.
- Manage Clinical Delivery reports and resource allocation across sites.
- Ensure accurate and compliant documentation in participants’ records.
- Coordinate care management services across various sites and in transition scenarios.
- Collaborate with Human Resources on staffing processes.
- Act as Director of Patient Services as assigned.
- Ensure compliance with all relevant regulations and standards.
Qualifications
- Degree from an NLN accredited nursing school (RN, BSN required or equivalent experience).
- Successful completion of an accredited Nursing Program required.
- Experience with infection control and case management is beneficial.
Experience
- Minimum of two years of administrative experience in a management capacity in a healthcare setting.
- Experience working with a frail or elderly population is beneficial.
- Supervisory experience is required.
Skills
- Strong communication and interpersonal skills.
- Proficiency in coordinating and managing interdisciplinary teams.
- Ability to maintain compliance with clinical and regulatory standards.
Education
- RN, BSN required or equivalent experience in a healthcare setting.
- Completion of NYSED-approved infection control coursework.
Additional Requirements
- Current active and unrestricted license as a Registered Nurse in New York State.
Schedule/Shift
Monday-Friday, 8:30am-5:00pm (1 hour lunch)
