Job Summary
Our client is seeking a Case Manager to provide intensive, trauma-informed case management to individuals living in Permanent Supportive Housing. This role primarily focuses on helping previously unhoused individuals achieve and maintain housing stability, wellness, and self-sufficiency. The position is temporary, with possibilities for extension or transition to a permanent role, depending on program needs and performance.
Responsibilities / Job Duties
- Provide intensive case management to newly housed individuals in Permanent Supportive Housing.
- Assist clients with applications for public benefits and accompany them to essential appointments.
- Support residents in understanding their lease and responsibilities as tenants.
- Coordinate care and referrals to mental health services and supportive resources.
- Engage residents in goal planning and skill-building activities to support housing retention.
- Contribute to a multidisciplinary team to foster a supportive and solutions-oriented environment.
- Maintain accurate documentation and case notes in agency systems.
Minimum Education and Experience Qualification Requirements
Education
- High School Diploma or GED
- Valid and up-to-date CA Driver's License and auto insurance
Qualifications
- Background in case management with experience working with vulnerable populations.
- Strong trauma-informed approach and ability to meet clients where they are.
- Excellent written and typing skills for clear documentation in case management systems.
- Ability to work independently in a collaborative, team-based environment.
Experience
- Minimum one year of experience supporting vulnerable populations (housing, healthcare, mental health, substance abuse).
Skills
- Compassion, patience, and understanding of competing priorities, including substance use and mental health challenges.
Schedule / Shift
Fully onsite - 8:30am to 5pm, Monday to Friday
