Nurse Practitioner (Adult / Family / Geriatric)
Location: Lancaster, PA
Schedule: Monday–Friday | 8:00 AM – 5:00 PM (typically wraps up around 4:30 PM)
Compensation: $120,000 – $130,000 annually

Position Overview
The Nurse Practitioner (NP) plays a vital role in delivering high-quality, patient-centered primary care to older adults enrolled in a PACE (Program of All-Inclusive Care for the Elderly) model. This position provides care both in a clinic-based setting and, as needed, in participants’ homes or partner facilities.
As a core member of the Interdisciplinary Team (IDT), the NP collaborates closely with physicians, social workers, therapists, dietitians, and other care team members to ensure seamless, coordinated care. The NP is responsible for developing and managing individualized care plans that align with each participant’s health goals, supporting independence, preventative care, and overall quality of life.

Key Responsibilities

Conduct comprehensive histories and physical exams for new and existing participants across care settings (clinic, participant homes, nursing facilities, hospitals, and other sites)

Evaluate and treat acute illnesses, changes in condition, and emerging symptoms

Develop, implement, and update individualized care plans in collaboration with the Interdisciplinary Team

Integrate primary care treatment plans into the broader IDT care strategy

Manage participant care in collaboration with Primary Care Physicians, including those residing in contracted facilities

Facilitate and coordinate referrals to medical specialists when clinically appropriate

Provide preventive health services, including immunizations, screenings, and routine monitoring

Prescribe, adjust, and manage medications in accordance with scope of practice and clinical guidelines

Order, review, and interpret diagnostic testing (labs, imaging, etc.)

Provide education and guidance related to therapeutic diets and care plans

Participate in infection control and surveillance activities alongside the Site Medical Director

Serve as a clinical resource for complex care management, geriatric syndromes, and illness/injury prevention

Travel to participant homes and partner facilities as needed (up to ~50% as census grows)
Patient Volume
Required Qualifications

Graduate of an accredited school of nursing

Current, unrestricted Nurse Practitioner license (Adult, Family, or Geriatric)

AANP or ANCC Board Certification

Active and up-to-date DEA license

Basic Life Support (BLS) certification (American Heart Association or American Red Cross)

Minimum of 2 years of experience as an Adult, Family, or Geriatric NP

Strong understanding of value-based care models, risk-based contracting, and Medicare/Medicaid regulatory frameworks

Proven ability to collaborate across disciplines in complex healthcare environments
