Job Summary
Our client is seeking a Licensed Clinical Social Worker to join their team. This role focuses on developing and managing individualized intervention plans for Medicare Dual Eligible Special Needs Plan (D-SNP) members. The Licensed Clinical Social Worker works with an interdisciplinary team to facilitate quality outcomes and promote optimal, achievable outcomes in a cost-effective manner. The position also involves participation in program evaluation and quality improvement efforts to ensure compliance with regulatory requirements.
Responsibilities / Job Duties
- Develop and manage individualized intervention plans for D-SNP members.
- Collaborate with the interdisciplinary Care Coordination team to facilitate quality outcomes.
- Educate members, caregivers, providers, and other stakeholders about navigating the managed care system.
- Participate in program evaluation and quality improvement initiatives.
Minimum Education and Experience Qualification Requirements
Education
- Master’s Degree in Social Work or related field.
Qualifications
- LCSW in CA.
- Two years of social service experience in an acute care, community-based, or managed care environment.
Experience
- Experience in developing and managing member-centered care plans.
Skills
- Familiarity with the principles of case management and coordination.
- Knowledge of community resources and behavioral health systems.
- Effective interviewing and culturally competent assessment skills.
- Proactive customer service and advocacy skills.
Schedule / Shift
Monday through Friday, 8am to 5pm, with some flexibility. Options include a 30-minute or 1-hour lunch and start times of 9am to 6pm or 7am to 4pm, ensuring coverage for business hours (8am to 5pm).
