Location: Seattle, WA (Integrated Primary Care)

Salary: $72,800 – $106,586 per year

Schedule: Monday – Friday | Core Hours: 7:30 AM – 6:30 PM

Key Responsibilities

• Clinical Therapy: Conduct psychosocial assessments and provide individual behavioral health therapy and counseling.

• Interdisciplinary Collaboration: Work alongside a team of providers to develop and coordinate comprehensive, "whole-person" care plans.

• Advocacy & Case Management: Link participants and families to community resources and advocate for needs regarding housing, mental health, and social stability.

• Care Transitions: Coordinate hospital discharges, residential placements, and transitions of care.

• End-of-Life Support: Provide compassionate guidance for end-of-life planning, grief counseling, and disenrollment.

Qualifications

• Licensure: Active Washington State LICSW (Licensed Independent Clinical Social Worker) is required.

• Education: Master’s degree in Social Work (MSW).

• Experience: 2+ years of behavioral health experience. Geriatric or dementia-focused experience is highly preferred.

• Skills: Proficiency in clinical documentation, knowledge of psychosocial issues in aging, and the ability to thrive within an interdisciplinary care model.

• Soft Skills: Empathy, cultural sensitivity, and strong organizational skills.
