Job Description
We are seeking a dedicated and resourceful Housing Navigator to join our Cal-AIM Community Supports team. In this role, you will work directly with Medi-Cal members to help them secure and maintain safe, long-term housing. Your focus will be on guiding individuals through the housing process, reducing barriers to stability, and connecting them with local support systems.
This is a highly collaborative position requiring coordination with Managed Care Plans (MCPs), Enhanced Care Management (ECM) providers, homeless service networks, and housing authorities.
Responsibilities/ Job Duties:
- Work one-on-one with clients to assess housing needs and create personalized housing plans
- Assist clients in navigating rental applications, housing program eligibility, and documentation
- Identify and build relationships with landlords, property managers, and housing providers
- Support clients in preparing for move-in, including inspections, deposits, and household setup
- Provide ongoing support to help clients retain housing and avoid evictions
- Conduct routine home visits and monitor housing conditions to ensure safety and stability
- Collaborate with care teams and external partners to ensure coordinated, wraparound services
- Maintain thorough documentation and case notes in line with contract and compliance standards
Minimum Education and Experience Qualification Requirements:
- Experience working in housing, homeless services, or case management (3+ years preferred)
- Familiarity with systems such as HMIS, CES, and housing assistance programs
- Knowledge of housing rights, landlord/tenant laws, and HUD guidelines
- Ability to work independently and manage a flexible caseload in the field
- Valid California driver's license and reliable transportation or access to transit
- Bilingual (Spanish/English) required
