Job Summary
Our client is seeking a dedicated Community Health Worker with experience in outreach for the CalAIM program. The candidate will be responsible for community engagement, enrollment of community members, and education about the program. The role involves working closely with patients to manage their care and ensuring they receive necessary services.
Responsibilities / Job Duties
- Engage in community outreach to enroll individuals in the CalAIM program.
- Provide education to the community to enhance awareness and enrollment.
- Manage a caseload of up to 50 clients, conducting assessments and handling referrals.
- Provide housing navigation services when needed.
- Coordinate with community-based settings to offer support and services.
- Ensure patient enrollment targets are met, with bonuses available for achieving higher enrollment.
- Drive to various community locations for outreach efforts.
- Set up outreach tables at clinics and community events.
Minimum Education and Experience Qualification Requirements
Education
- Bachelor's Degree in Social Work, Psychology, Sociology, or related field preferred.
- Four to five years direct experience required if lacking a bachelor's degree.
Qualifications
- Community Health Worker Certification is highly preferred.
- Bilingual in English and Spanish is preferred.
Skills
- Experience with outreach programs, ideally the CalAIM program.
- Strong independent working ability with minimal supervision.
- Capability to drive to and work in various locations independently.
Additional Requirements
Mileage reimbursement for traveling within the community to set up enrollment tables.
Schedule / Shift
Monday to Friday, 6 hours per day, between 8 am and 3 pm.
