Job Summary

Our client is seeking a dedicated and experienced Pharmacist to join their team. The primary responsibilities for this position include assessing patient suitability for home infusion therapy, designing pharmacy care plans, and recommending or adjusting medication therapies based on clinical data. The role also involves preparing, compounding, labeling, and dispensing medications, conducting nutritional assessments, and collaborating with other healthcare professionals to ensure continuity of care.

Key Responsibilities

• Assess patient suitability for home infusion therapy.
• Design, monitor, and evaluate individualized pharmacy care plans.
• Recommend or adjust medication therapies based on clinical data.
• Prepare, compound, label, and dispense medications per USP and DEA standards.
• Conduct nutritional assessments and provide patient/caregiver education.
• Collaborate with nursing and other departments to ensure continuity of care.
• Monitor controlled substance inventory and report discrepancies.
• Provide drug information to healthcare professionals and patients.
• Participate in CEU and professional competency programs.
• Adhere to our client’s policies, quality assurance, and regulatory requirements.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Active Pharmacy License in the State of OK.
• 1+ year experience in sterile IV admixture procedures.
• Proficient in Microsoft Word, Excel, and PowerPoint.
• Strong customer service and patient-care orientation.
• Preferred experience in home care, hospital, or clean room settings.

• Paid Sick Leave (Medix provides paid sick leave according to state and local sick leave ordinances).
