Clinics are located in accessible, retail-style settings rather than traditional medical office complexes, creating a modern, patient-friendly environment and a collaborative workspace for clinical teams.
This is a newly created role designed to expand acute care access within a value-based primary care model.
Position Overview
We are seeking an experienced Acute Care Nurse Practitioner (ACNP) to support acute and urgent patient needs while collaborating closely with Primary Care Providers to maintain continuity of care.
This role blends acute care with outpatient primary care responsibilities and is ideal for a provider who enjoys variety, clinical autonomy, and working within a team-based, patient-centered model.
Key Responsibilities

Provide outpatient acute and primary care services for a senior patient population

Evaluate and manage walk-in visits, sick visits, hospital follow-ups, and urgent clinical needs

Work closely with patients’ Primary Care Providers to ensure continuity and coordination of care

Support Registered Nurses with patient triage, including phone-based clinical guidance

Participate in clinical meetings, including hospitalized patient reviews, quality improvement initiatives, outcomes review, and peer review

Maintain accurate, timely, and compliant medical documentation

Collaborate with Medical Assistants, care teams, and operational leadership to deliver high-quality care

Assist with identifying patients who require outreach or acute intervention

Participate in community outreach or engagement events as needed

Provide on-call coverage for nights, weekends, and holidays based on a shared call schedule consistent with other providers

Travel within the local market to nearby centers as needed

Training and Onboarding

Training length and structure are individualized based on experience

Online training begins prior to clinic opening

New colleague orientation typically occurs within the first 90 days

Orientation and leadership exposure may occur in Nashville or Phoenix, with organizational leadership involvement

Competitive base salary: $125,000 – $160,000

Required Qualifications
-
Acute Care Nurse Practitioner (ACNP) certification
-
Active, unrestricted Nurse Practitioner license in Missouri or in process
-
Minimum of 2–3 years of Nurse Practitioner experience (not open to new graduates)

Experience caring for elderly or Medicare-aged patients

Background in internal medicine, family medicine, geriatrics, or managed care preferred

Current BLS and ACLS certifications

Strong clinical assessment and diagnostic skills

Excellent communication and collaboration abilities

Comfort managing medically complex patients with multiple comorbidities
