Job Summary
Our client is seeking a dedicated Social Worker who will undertake a dual role, working with both ambulatory clinic patients and rounding on the inpatient stem cell transplant unit. The primary responsibilities include conducting initial assessments for patients moving toward transplant, identifying any assistance needed, and coordinating care across the inpatient and outpatient spectrum.
Key Responsibilities
- Conduct psychosocial assessments for admitted patients in the inpatient unit.
- Provide emotional support and crisis intervention.
- Address discharge barriers and coordinate post-acute resources.
- Facilitate communication between inpatient and outpatient teams.
- Complete pre-transplant psychosocial evaluations for new candidates in the outpatient setting.
- Provide ongoing counseling and caregiver support during treatment and survivorship.
- Coordinate with inpatient social workers for admissions and re-admissions.
- Manage outpatient resources, housing, and follow-up needs.
Qualifications
- MSW Required
- Acute care social work experience
- Crisis management skills
- Experience in discharge planning and interdisciplinary collaboration
- Proficiency in psychosocial evaluation and caregiver education

Additional Requirements
The position requires flexibility in schedule, with typical hours from Monday to Friday, 8 AM to 5 PM, but allows for flexible start and end times.
