Job Summary
Our client is seeking a highly qualified Nurse Practitioner to work within a Medicare Advantage Value-Based Care Model, serving the senior population. The role involves building meaningful relationships with patients by allotting 45 minutes for each new visit. The Nurse Practitioner will work alongside a doctor, focusing on non-complex cases, prescribing medicines, and prioritizing patient care over volume.
Key Responsibilities
- Prescribe medicine and address non-complex cases.
- Develop strong relationships with senior patients through extended appointment times.
- Adopt value-based care principles aligned with Medicare Advantage, ensuring patient-centered care.
- Follow a full schedule that includes a daily team huddle, seeing patients, and end-of-day administrative tasks.
Qualifications
- BLS or ACLS Certification required.
- NP License in the state or in process of obtaining it.
- Family NP experience preferred over hospital NP experience.
- Several years of Primary Care experience (including Geriatrics) or Value-Based Care (VBC) experience.
Experience
- 2-3 years of experience in primary care, ideally with value-based care experience or a strong emphasis on geriatrics.
- Experience working with the elderly patient population and managed care.
Skills
- Proficiency with eClinical Works and familiarity with value-based care models.
- Strong interpersonal and communication skills for building patient relationships.
Additional Requirements
- Willingness to participate in orientation and training within the first 90 days, potentially in multiple locations nationwide.
