Job Summary
Our client is seeking a Case Manager who will be responsible for conducting comprehensive assessments, developing and managing individualized care plans, conducting member outreach, and ensuring documentation and compliance. Primary goals include identifying members who may require case management, creating care plans in collaboration with medical professionals, and providing educational support to members.
Responsibilities / Job Duties
- Complete comprehensive assessments by evaluating members’ medical, social, and behavioral needs.
- Identify members requiring case management using clinical judgment and assessment triggers.
- Develop and manage individualized care plans with members, caregivers, physicians, and other care professionals.
- Continuously monitor, update, and adjust care plans as needed based on member progress.
- Conduct regular telephonic and virtual/in-person visits and maintain an active caseload.
- Provide care coordination and support continuity across medical and behavioral services.
- Educate and support members using motivational interviewing and clinical guidelines.
- Document assessments, care plan updates, and interventions according to required standards.
Minimum Education and Experience Qualification Requirements
Education
- Must hold either RN or LVN credential in California.
Qualifications
- Must be bilingual in Korean.
- 2-3 years of experience in case management, disease management, managed care, or medical or behavioral health settings preferred.
- High level of critical thinking and problem-solving skills.
- Effective communication skills including verbal and written.
- Ability to manage time effectively and work independently in a fast-paced environment.
- Strong work ethic and overall positive attitude.
- Demonstrated flexibility, organization, and self-motivation.
Experience
- Medicare Case Management experience and knowledge of SNP programs are highly preferred.
Skills
- Must be either an LVN or RN in CA.
- Must be bilingual in Korean.
Schedule / Shift
- Type: Full time, Direct Hire
- Schedule: M-F 8am-5pm
Additional Requirements
- Primarily work from home but must be flexible to attend member events about 1-2 times per quarter in Southern California.
- Must be local to either East LA or North Orange County.
