Job Summary
Our client is seeking a dedicated Medical Surgical Registered Nurse to provide comprehensive case management and discharge planning. This position requires strong clinical expertise in a dynamic healthcare environment, ensuring effective patient care coordination and timely discharges.
Key Responsibilities
- Daily Unit Integration: Report daily to assigned Med-Surg units and integrate swiftly into unit structure, staff, and workflows.
- Rapid Assessment and Prioritization: Review daily caseload to identify high-risk and complex cases for intervention.
- Protocol Flexibility: Navigate and adhere to various unit-specific protocols and documentation processes efficiently.
- Comprehensive Patient Assessment: Perform assessments to determine patient needs and facilitate safe, timely discharges.
- Discharge Planning Execution: Develop and implement individualized transition plans, coordinating necessary services.
- Interdisciplinary Team Collaboration: Lead daily interdisciplinary rounds to support discharge decisions and manage length of stay.
- Patient and Family Education: Serve as a central communication point and educate patients and families on discharge plans and rights.
- Status Management: Collaborate with physicians to ensure correct admission status and manage potential denials or appeals.
- Documentation and Quality: Ensure all documentation complies with hospital, state, and federal regulations while participating in quality improvement efforts.
Qualifications
- Minimum 5-7 years of solid clinical nursing background.
- Minimum 1 year of experience in Inpatient Hospital Case Management or a comparable leadership role, focusing on complex cases.
- Active and unrestricted Maryland Registered Nurse (RN) License.
- Preferred: Case Management certification (e.g., CCM, ANCC) to be obtained within 9 months of hire.

Education
- Bachelor of Science in Nursing (BSN) or higher degree.
