Job Summary
Our client is seeking a Complex Case Manager to independently manage a specialized caseload of approximately 15 adult medical/surgical patients, ensuring seamless transitions of care. This advanced, high-impact role focuses on leveraging clinical expertise and system navigation skills to optimize patient outcomes.
Key Responsibilities
- Independently manage a specialized caseload of approximately 15 adult medical/surgical patients with complex health needs.
- Lead complex discharge planning and care transitions to ensure seamless patient movement across the care continuum.
- Collaborate closely with social workers, care management leadership, and senior physician advisors.
- Identify and resolve systemic barriers to discharge and serve as a departmental representative in various communications.
- Lead departmental initiatives or committees to influence the health system positively.
- Maintain flexibility and critical thinking to adapt to evolving institutional directives and strategic priorities.
Qualifications
- Active and unrestricted Maryland Registered Nurse (RN) License.
- Bachelor of Science in Nursing (BSN) or higher degree.
Experience
- Minimum 5-7 years of solid clinical nursing background.
- Minimum 3 years of experience in Inpatient Hospital Case Management or comparable leadership role, focusing on managing highly complex patient cases.
Skills
- Demonstrated professional acumen and leadership qualities.
- Polished, professional communication skills for engaging with senior leadership.
Additional Requirements
- Certification in Case Management (e.g., CCM, ANCC) preferred upfront. Mandatory to obtain certification within 9 months of hire.
- Full-time Schedule: Monday–Friday, 8:30 AM – 5:00 PM (Business Casual attire).
