Job Summary
Our client is seeking a Medical Assistant committed to providing world-class service to every member. As a Connected Care Assistant, you will work alongside the clinical team, helping members manage complex or chronic health conditions by coordinating patient-care services, providing support, and delivering additional services as identified.
Key Responsibilities
- Focus on member engagement by completing scheduled shifts for member calls, chats, and internal referrals.
- Conduct targeted outreach shifts to engage at-risk members.
- Use autodialer tools for outreach to eligible populations.
- Manage and triage related care plan task queues.
- Coordinate care for members and their families as identified by the clinical team.
- Recommend benefits available to members through their employers.
- Initiate related cases as identified by the clinical team.
- Complete administrative tasks including composing letters, obtaining medical records, and monitoring communications.
- Attend training courses to stay current with healthcare developments.
- Comply with organizational and healthcare guidelines.
Qualifications
- Prior experience in case management or healthcare setting is preferred.
- Proficiency in motivational interviewing techniques.
- Strong telephone skills and effective communication skills, both verbal and written.
- Effective time management and ability to work autonomously.
- Highly organized with the ability to manage tasks and meet deadlines.
Skills
- Proficient in computer technology and health IT systems.
- Ability to multitask and remain flexible.
- Knowledge of community healthcare and social welfare resources is preferred.
- Positive and respectful attitude towards peers, team members, and patients.
