Job Summary
We’re seeking Nurse Care Managers to join our client's Care and Case Management team. The primary focus of this role is delivering holistic care to members through their healthcare journey. The telephonic Nurse Care Manager will collaborate with a multidisciplinary team to provide integrated remote care, guide members through complex medical situations, and create cohesive care plans. This role involves extensive phone communication, advocating for members, and ensuring effective care management for excellent health outcomes.
Key Responsibilities
- Deliver coordinated, patient-centered virtual Care Management via telephone or video to improve health outcomes.
- Generate impactful care plans in collaboration with members and multidisciplinary care teams.
- Assist members in navigating complex medical conditions and healthcare systems.
- Partner with local providers to ensure coordinated care.
- Provide compassionate, longitudinal follow-up care and build supportive relationships.
- Support members through acute healthcare episodes with coordinated Case Management.
- Coordinate necessary resources that address both clinical and social issues.
Qualifications
- Bachelor of Science in Nursing (BSN).
- Current CCM Certification required.
- Must reside in a compact NLC state with an active Compact RN license.
- Active California Nursing License preferred.
- Willingness to become licensed in multiple states.
- 2+ years experience in care management.
- Spanish speaking desirable.
Experience
- 5+ years of experience in nursing preferred.
- Experience working remotely and comfort with technology.
- Strong competence in using multiple computer/medical record systems.
Skills
- Comfort discussing a wide variety of medical conditions.
- Highly empathetic with a focus on patient reassurance.
- Efficient with good judgment for prioritizing tasks.
- Flexible and comfortable in rapidly-changing environments.
- Strong verbal and written communication skills.
