Job Summary
Our client is seeking a dedicated Community Health Worker to join their team. The primary responsibility of this role is to conduct hospital-focused outreach activities, providing direct support to members experiencing homelessness. The goal is to improve health outcomes by connecting individuals with essential housing and healthcare services.
Responsibilities / Job Duties
- Perform consistent and frequent outreach services in assigned hospitals/healthcare systems.
- Build relationships with discharge planning staff.
- Establish and maintain communication procedures for accepting referrals from hospitals/healthcare systems.
- Contact homeless individuals prior to discharge to introduce available resources.
- Enroll patients in support programs and facilitate their access to additional resources.
- Collaborate with existing providers to prevent duplication of efforts or services.
- Assist the Program Manager by developing reports for community stakeholders quarterly.
- Provide crisis intervention and support to homeless individuals.
- Assist clients with obtaining necessary documents such as ID cards and social security income.
- Identify and assist clients in accessing temporary and permanent housing options.
- Survey the rental market for affordable housing and advocate for clients with landlords.

Additional Requirements
- Driving is an essential function of this position, requiring reliable transportation.
- Candidate must have a valid driver's license.
- Proof of insurance and vehicle ownership required for work purposes.
