Job Summary
Our client is seeking a Triage Registered Nurse to provide expert patient interaction and care coordination. The role involves assessing patient symptoms, collaborating with medical teams, coordinating procedures, and providing tailored patient education. The nurse will also be responsible for maintaining accurate documentation and supporting quality improvement initiatives to ensure optimal patient outcomes.
Key Responsibilities
- Receive and respond to patients’ clinical inquiries via telephone, email, or in person in a timely and compassionate manner.
- Assess patient symptoms and concerns, determining the appropriate level of care and delegating accordingly.
- Collaborate and communicate effectively with physicians, electrophysiologists, advanced practice providers, medical assistants, and back-office teams to coordinate patient care.
- Facilitate multidisciplinary team interactions to enhance patient-centered care and ensure seamless care transitions.
- Assist in the coordination of cardiology and/or electrophysiology procedures, including pre-procedure preparation, post-procedure follow-up, and patient education.
- Provide patient education tailored to individual learning styles regarding pre- and post-procedure expectations, medication management, and lifestyle modifications.
- Review, communicate, and educate patients on laboratory results, imaging studies, medication refills, and medication adjustments as per protocol and physician directives.
- Complete necessary documentation, including cardiac clearance letters, FMLA/disability forms, cardiac rehabilitation referrals, and home health orders.
- Review external medical records and update electronic medical records appropriately to ensure accuracy and continuity of care.
- Conduct and document medication reconciliation and update patient medical, surgical, and social histories as needed.
- Maintain accurate documentation of patient plans of care and interventions in the electronic medical record per policy and regulatory requirements.
- Maintain competency in Basic Life Support (BLS), infection control, safety protocols, and required skills training.
- Implement evidence-based interventions in accordance with research and best practices in cardiology and electrophysiology.
- Support and participate in quality improvement initiatives to optimize patient outcomes in outpatient cardiology and electrophysiology care.
- Perform other duties as assigned to support clinic operations and patient care.
Qualifications:
- Registered Nurse License
- Bachelors of Science - Nursing
- 2-3 years of nursing experience
