Job Summary
Our client is seeking a Risk Adjustment Coder responsible for ensuring compliance with applicable laws and regulations related to coding and documentation. The primary goal is to conduct risk adjustment reviews, assess medical records, provide feedback on documentation standards, and improve individual member risk adjustment score accuracy.
Key Responsibilities
- Ensure compliance with Federal, State, and County laws related to coding and documentation guidelines for Risk Adjustment Reviews.
- Verify that diagnosis codes are supported by documentation according to ICD-10-CM Guidelines.
- Capture the correct diagnosis codes for chronic or major medical conditions.
- Query unsubstantiated diagnosis codes and review clinical indicators for accuracy.
- Conduct medical chart reviews to identify improvement opportunities for risk adjustment score accuracy.
- Provide feedback on documentation and physician coding standards to internal clients.
- Identify clinical coding trends and training needs to enhance documentation quality.
- Attend all required meetings.
