Job Summary
Our client seeks a Member Coordinator (MC) who ensures each member receives compassionate, timely, and coordinated support across various care programs including Enhanced Care Management (ECM) and Community Supports (CS). This high-touch healthcare admin position requires empathy, documentation accuracy, and quick responsiveness in a fast-paced, live handoff environment.
Responsibilities / Job Duties
- Conduct trauma-informed, full-length assessments to understand member needs, eligibility, and care pathways.
- Document and input all assessment data into EMR/CRM.
- Coordinate with Lead Care Managers (LCMs) to initiate services.
- Verify Medi-Cal/insurance eligibility and demographic data across systems.
- Ensure EMR and tracking systems are updated daily.
- Manage cross-referencing between portals and maintain HIPAA compliance.
- Manage live call follow-ups or callbacks as needed.
- Link members to appropriate services or community-based resources.
- Manage non-responsive members and attempt to re-engage via multi-channel outreach.
- Conduct self-audits and peer reviews for documentation quality.
- Identify workflow issues and recommend process improvements.
- Support with authorizations, faxes, record updates, and tracking member communications.
Skills
- Proficiency with Monday.com, EMR/CRM systems, Microsoft 365, and Google Workspace.
- Strong phone, text, and email outreach skills.
- Experience with HIPAA-compliant documentation tools.
