Job Summary

Our client is seeking a dedicated Community Health Worker to join their team. The primary goal of this role is to provide comprehensive care coordination for an assigned patient caseload, ensuring integrated care services are targeted, appropriate, and beneficial. This involves collaborating with patients, families, caregivers, and healthcare providers to meet complex social needs.

Key Responsibilities

• Provide comprehensive care coordination to a patient caseload.
• Collaborate with patients, families, caregivers, and healthcare providers to meet complex social needs.
• Promote communication and collaboration among healthcare team members.
• Intervene regarding emotional, social, and financial consequences of illness/disability.
• Conduct in-person visits to patient homes as needed.
• Document interventions in patient medical records timely and accurately.
• Onboard patients to the medical and social care model.
• Provide patient education on disease management.
• Establish healing relationships with patients and families.
• Communicate with patients using various methods including in-person, phone, and messaging.
• Coordinate care across the continuum including scheduling and referrals.
• Participate in daily operations of a primary care practice.
• Track patient enrollment and progression through care programs.
• Other duties as assigned by the Guia Manager.
