Job Summary

Our client is seeking an RN Case Manager for Infectious Disease to support the provision of coordinated medical and integrated behavioral health services within the Harm Reduction Program. This role involves working with patients who have HIV, TB (active and latent), Hepatitis B and C, other sexually transmitted infections, and those who access Syringe Services. The RN Case Manager will manage a caseload of 60-70 patients, ensuring they receive necessary scans, tests, and follow-up care via phone and telehealth. The RN will work independently and collaborate closely with IV labs, patients, and the state in a state-funded TB clinic.

Key Responsibilities

• Manage a caseload of 60-70 patients in the Harm Reduction Program.
• Coordinate and provide integrated behavioral health services.
• Ensure patients undergo necessary scans and tests prior to visits.
• Conduct follow-up appointments via phone and telehealth.
• Collaborate with IV labs, patients, and the state for TB management and treatment plans.
