Job Summary
Our client is seeking a conscientious and motivated Care Manager to work collaboratively within a multidisciplinary team of healthcare professionals. The position involves implementing individualized treatment plans, ensuring care coordination and linkage to mental health and primary care services with the aim of improving health outcomes for individuals with serious mental illnesses. The role requires fostering an innovative environment focused on excellence in service delivery.
Responsibilities / Job Duties
- Conduct strengths-based mental health rehabilitation services and case management assessments gathering comprehensive client information.
- Educate clients on health management including chronic disease conditions such as diabetes and hypertension.
- Coordinate care by tracking clients and facilitating appointments, and referrals to necessary health and social support services.
- Maintain complex caseloads and provide ongoing care management and educational support.
- Develop and maintain communication with treatment providers and family members to ensure continuity of care.
- Lead by example, demonstrating professional ethics and commitment to agency values.
Minimum Education and Experience Qualification Requirements
Education
- Bachelor Degree in Behavioral Health.
Experience
- Minimum of two years experience in a health and human services field working with clients who are persistently and severely mentally ill preferred.
Skills
- Knowledgeable in primary care, chronic disease management, and health-related issues.
- Strong communication skills to engage effectively with a culturally diverse consumer population.
- Proficiency in developing and maintaining comprehensive documentation and client management records.
- Ability to implement and evaluate therapeutic interventions and manage client self-care education.
