Job Summary
We are seeking a Health Care Assistant who is committed to providing world-class service to our client's members. In this role, you will support the clinical team to help members manage their complex or chronic health conditions. Your responsibilities will include coordinating patient-care services, providing support, and identifying additional services required by the clinical team. The ideal candidate should possess a compassionate nature, relevant healthcare knowledge, and exceptional customer service skills.
Key Responsibilities
- Focus on member engagement and complete scheduled shifts for member calls, chats, and internal referrals.
- Conduct targeted outreach to engage at-risk members using autodialer tooling.
- Manage and triage related care plan task queues.
- Coordinate care for members and their families/caregivers as identified by the clinical team.
- Recommend available benefits to members through employer benefits navigation.
- Initiate related cases such as Provider Match, Concierge Referrals, and Expert Medical Opinion.
- Complete various administrative tasks including composing letters and obtaining medical records.
- Triage callback tickets, referrals, and escalations effectively.
- Monitor email referrals, communications, and manage relevant portals.
- Attend team huddles and ongoing training sessions when requested.
- Comply with organizational guidelines and adhere to healthcare laws and regulations.
Qualifications
- Prior experience in case management or healthcare setting is preferred.
- Proficiency in motivational interviewing techniques.
- Ability to multitask and remain flexible in a constantly changing environment.
- Strong telephone skills, including navigating conversations and anticipating needs.
- Professional and effective communication skills, both verbal and written.
- Effective time management and autonomy in work.
- Highly organized with strong task and deadline management abilities.
- Proficient in computer technology and quick to learn new platforms.
- Desirable experience with health IT systems and generating reports.
- Preferred knowledge and connections to community healthcare and social welfare resources.
- Positive and respectful attitude towards peers, team members, and patients.
- Maintains patient confidentiality at all times, adhering to HIPAA guidelines and regulations.
