Medix is seeking a Clinical Pharmacist with expertise in cardiology and ambulatory care. In this role, you will work closely with the cardiology team to ensure patients receive appropriate medications, education, and counseling to manage their cardiovascular health. You will provide expert pharmaceutical care, assist with medication management, and collaborate with healthcare providers to optimize patient outcomes. This role allows you to make a direct impact on patients with heart disease and related conditions.

Responsibilities:

• Medication Management: Review and manage cardiology patient medication regimens for safety, efficacy, and adherence.

• Patient Counseling: Educate patients on proper medication use, side effects, and potential interactions, focusing on cardiovascular drugs.

• Collaborative Care: Work with cardiologists, nurses, and other providers to create and adjust patient treatment plans.

• Clinical Support: Monitor patient progress, recommend medication adjustments, and assist with interpreting lab results.

• Education & Training: Provide ongoing education to staff and patients on cardiovascular therapies and guidelines.

• Pharmacovigilance: Monitor and report adverse drug reactions and interactions.

• Patient Advocacy: Ensure patients understand their medications, lifestyle modifications, and treatment options.

Qualifications:

• PGY1 residency, required

• Ability to work independently

• Baccalaureate degree or PharmD from an accredited school of pharmacy

• Current, unrestricted South Carolina pharmacy license

Required Skills:

• Expertise in clinical pharmacy with a cardiology focus

• Strong communication and collaboration skills

• Ability to work independently within healthcare teams

Preferred Skills:

• Experience in a cardiology setting

• Familiarity with cardiovascular medications
