Job Title: Care Management Director

Location: hospital, onsite in Erie, PA

Ideal Profile: Director at another hospital, large nursing home or a manager from another hospital

Overview

• For nursing, must have bachelor’s degree in nursing (BSN)
• 3 years of hospital based case management experience, including utilization review and discharge planning experience
• Candidate must obtain CCM or ACM certification within the first year (can obtain without being a licensed social worker, if a social worker, must have Masters in social work)
• State licensure required per state if RN or social worker (RN preferred)
• 7 direct reports: 4 FT case managers (2 RNs and 2 social workers), 1 PT case manager assistant, 2 per diem case managers
• 60 bed hospital

Pay:

• Salary range is 100-112k plus eligibility for quarterly bonus
• Offering $5k sign on bonus

Schedule: Mon-Fri, 40 hours per week. However, the hospital operates 24/7, therefore the Sr. Team typically rotates every 8-10 weeks to do on-call on weekends.
